Assist-Personal Activities
Provide assistance with daily personal tasks, including bathing, dressing, grooming, and mobility, to promote independence and enhance quality of life.

Assist-Travel/Transport
Facilitate access to transportation for community, social, and work activities, ensuring participants can travel safely and independently.

Home Modification
Implement custom-built changes to a participant’s home, such as installing ramps or modifying bathrooms, to improve accessibility and independence.

Community Nursing Care
Deliver skilled nursing services to participants in their homes, including wound care, medication management, and health monitoring, to support medical needs and well-being.

Daily Tasks/Shared Living
Assist with routine household tasks and shared living arrangements, promoting independence and community integration through support with daily activities.

Innov Community Participation
Encourage and support participants to engage in innovative community activities, fostering social inclusion and skill development through creative programs.

Development-Life Skills
